We encourage all patients to have online access in order to easily request repeat prescriptions, and to make or cancel appointments. If you would like to use this service, please register for online services.

Under General Data Protection Regulations (GDPR), you have a right to receive a copy of your records by making a Subject Access Request.

Please ask at reception if you would like to make a Subject Access Request, or to discuss online access as this is often suitable for most needs.

A Subject Access Request is free of charge, however subsequent copies and repeated requests within a short period of time will incur an administrative cost.

You will be asked to prove your identity when making the request, and can receive your record via email (password-protected attachment) or as a print-out.
