We welcome new patients at the practice and hope to make the process of registration with us as simple as possible. You do not need to make an appointment to register at the practice - you can just call in when we are open.
As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor.
Medical treatment is available from the date of registration. Please contact reception for further information or print and complete the new patient registration form by clicking on the following link. Once completed bring it in to reception. You may be asked to provide proof of residency so be prepared to bring your passport, proof of address, EHIC, Provisional Replacement card or S1 form.
We require the two forms below to be completed to register you as a patient:
You can download the GMS1 form and bring it in with the necessary documentation, or ask for a GMS1 form at the Reception desk and complete it in the waiting room.
Information you will need includes your previous address details and the name and address of your previous GP.
Please bring relevant documentation with you including photo I.D. (such as driving licence, passport) and proof of address (such as bank or utility statement) - as this will speed up your registration process. If you are unable to provide proof of your address we will still register you at the surgery.
All patients seeking to register need to have their own GMS1 form and present themselves to Reception - including children and babies.
The second part of registering as a new patient is completing the New Patient Questionnaire. This will ensure the safe transfer of your care, as it may be several weeks before we receive your complete medical records. Please click on the appropriate link below to print.
